UMC is proud of our advanced illness nurses, who work hard to alleviate the hardships for patients who suffer from a serious illness such as cancer, heart failure, liver disease, and more. These nurses collaborate with physicians and nurses in the emergency center and in the hospital to make the advanced care planning process as smooth as possible.
It is important that patients with a shorter life expectancy discuss their goals of care and have the opportunity to seek the benefits from advanced care planning. Our advanced illness nurses team step in and assist with the revision of care plans and discuss things such as the medical power of attorney and DNR (do not resuscitate) status. They also make necessary referrals and visit the patient in the hospital to make sure the care plan is being carried out, while working closely with our supportive care team.
